How to fill in the header of the sole decision to create an LLC?
The decision of the sole participant (founder) of the Limited Liability Company “NAME” (hereinafter referred to as the Company).

What to write about the founder?
The founder is present: Surname, First name, Patronymic, date of birth, passport data (Series, number (a unique entry in the demographic register of Ukraine), by whom, when issued), index and address of residence registration, identification code.
What to write in the decision of the participant?
Decided:
- Create Limited Liability Company “TITLE”. Below you need to add short name LLC “TITLE”;
- Form the authorized capital of the Company in the amount of 100,000 (one hundred thousand) hryvnia, 00 kopecks;
- Approve the size of the parts of the participants in the authorized capital (when there is one founder, all one hundred percent of it);
- Approve the procedure for making contributions by members of the Company (usually they write: pay the authorized capital of the Company at the end of a six-month period from the date of state registration);
- Approve the location of the Company: index, legal address in full with the number of the house and office;
- The subject of the Company’s activity is the formation of a market for goods and services, not prohibited by the current legislation, for the purpose of making a profit. The Company receives permits (licenses) for certain types of activities, if it is provided for by the relevant legislation in force in Ukraine. The implementation of activities by the Company, which requires an appropriate permit (license), is carried out after obtaining such a permit (license) in the prescribed manner. The Company, in accordance with the established procedure, carries out foreign economic activity within the framework of the subject of its activity. According to Codes of types of economic activity in Ukraine, the following activities were selected: 62.01 Computer programming (main), 61.90 Other activities in the field of telecommunications, 62.02 Consulting on informatization, 62.03 Activities in the management of computer equipment, 62.09 Other activities in the field of information technology and computer systems, 63.11 Data processing, placement information on websites and related activities, 63.12 Web portals, 68.20 Renting and operating own or leased real estate, 77.33 Lease of office machines and equipment, including computers, 47.41 Retail sale of computers, peripheral equipment and software in specialized stores, 46.51 Wholesale of computers, peripheral equipment and software;
- Approve the charter (own charter document) of the Limited Liability Company “TITLE”;
- Approve the Supreme Body of the Society – the General Meeting;
- Approve the executive body of the Company – Director (CEO);
- Appoint the director of the Company: Surname First name Patronymic, date of birth, passport data (Series, number (a unique entry in the demographic register of Ukraine), by whom, when issued), index and address of registration (registration), identification code.
How to complete the sole decision to create an LLC?
Further, at the statutory meeting of the founders (participants) of the Company, no additional issues were considered, and the meeting was closed.
